Special Needs Registry

The Special Needs Registry is designed to provide critical information to emergency responders about persons with special needs. Our goal is to accurately maintain the registry to better serve anyone with special needs within the Village of Shorewood. The Special Needs form can be picked up at the Shorewood Police Department Monday through Friday, 8:00 a.m. to 4:30 p.m., faxed or emailed to you, or click on the icon below.
